Tirzepatide (Zepbound)
- Titration
- Weekly subcutaneous injection, starting at 2.5 mg (a treatment-initiation dose, not a maintenance dose). After 4 weeks, increased to 5 mg, then optionally in 2.5 mg increments every 4+ weeks up to a maximum of 15 mg.
- Maintenance dose
- 5 mg, 10 mg, or 15 mg once weekly — many patients stay at 5 mg or 10 mg rather than titrating to the maximum.
- Trial results
- In the SURMOUNT-1 trial (72 weeks), participants on the 15 mg dose lost a mean of 20.9% of body weight, vs. 3.1% on placebo.
